Baptism Request Form
Once you have decided to believe and follow Jesus, Baptism is to be the
next step in your journey with the Lord. The waters of baptism symbolize
the forgiveness and cleansing God grants to those who have faith in
Jesus. These waters are also symbolic of a liquid "grave" of the old
man, and the "resurrection" of a new creature in Christ.
The Covenant Church offers multiple opportunities throughout the year
for people to be baptized in one of our services. If you would like to
be baptized, please fill out the following form and an Elder on our
Ministerial Staff will contact you with the class schedule and
confirmation date of the dedication; as well to confirm your family's
participation in the ceremony.

James records, "Is any sick among you? Let him call for the elders of the church; and let them pray over him, anointing him with oil in the name of the Lord: And the prayer of faith shall save the sick, and the Lord shall raise him up." James 5:14-15
If you or a loved one who lives in the Atlanta metropolitan area would like visitation from one of our ministry volunteers, please complete this form.
Thank you for submitting a prayer request to The Covenant Intercessory Prayer Ministry. This form notifies our intercessors to pray for you and your need.
The Lord moves when His people come together in agreement. Jesus said, "If two of you shall agree on earth as touching anything that they shall ask, it shall be done for them of my Father which is in Heaven. For where two or three are gathered together in my name, there am I in the midst of them." Matthew 18:19-20.
When you submit your prayer request, remember Jesus' promise: "And whatsoever ye shall ask in my name, that will I do, that the Father may be glorified in the Son. If ye shall ask any thing in my name, I will do it." John 14:13-14.
Please briefly share your request by clicking the link above. Include your name, contact information and prayer request. Please be as brief as possible, but provide all necessary details to properly guide our prayers.Covenant Postal Mailing List
